Become a patient
We are delighted that you are interested in becoming a patient of Parker
Internal Medicine.
Please follow these steps to initiate the process:
1. Call us at 704-384-1605 to set an appointment. Let us know of active
problems that will require attention at the time of the visit (such
as hypertension, diabetes, thyroid disease) or whether you just need
to have a visit to establish a relationship. This will help us to budget
your time appropriately so that we do not have to rush through active
problems.
2. If you have a significant medical history, contact your prior-treating
physician prior to your appointment so that you can authorize him to
advance your records to our office before your appointment. You can
download a records release form
and advance that to him well before your appointment. This will allow
me to have access to important information prior to or at the time of
your appointment that may affect the decisions that we make at the initial
visit.
3. At the time of your appointment, please bring all of your medications,
a list of your allergies, insurance information and the copayment amount.
Download and complete the medical
history questionnaire available on our site and bring it with you
to the initial appointment.
4. You can download the registration
form from our site, complete it before your appointment and bring
it with you.
5. Show up for your appointment a few minutes early. We are working
diligently to stay on time since so many of our patients operate on
a tight schedule with business and family responsibilities.

6. If you are unable to make your appointment please notify us at least
24 hours in advance. Repeated cancellations or failures to show up for
appointments make it difficult for us to provide excellent service.
7. Let us know what we can do to improve our service.
